EVALUATIONS

Evaluations are conducted for professional boards, physicians, and attorneys when there is question of a mental health or substance use disorder.  Sometimes, individuals request an evaluation after having been in counseling that has been less than successful.  

 

Dr. Maynard tests for:

  • Mood distoders (e.g., depression, bipolar, etc),

  • Psychotic disorders (e.g., schizophreniform, delusional, etc),

  • Alcohol and substance use disorders,

  • Anxiety disorders (e.g., social anxiety, generalized anxiety, etc),

  • Posttraumatic stress disorder,

  • Attention-Deficit/Hyperactivity Disorder,

  • and the Personality Disorders.

Dr. Maynard utilizes various testing instruments to assist in determining whether or not a disorder exists.  Testing instruments frequently used include:

  • SCID-5-CV,

  • SCID-5-PD,

  • MMPI-3,

  • NEO-PI-3,

  • AUDIT,

  • Brown ADD,

  • DAST,

  • PTSD Check List

  • TSI-2

  • and others

 

Dr. Maynard’s evaluations have been submitted to various entities and professionals, such as:

  • Kentucky Board of Medical Licensure,

  • Kentucky Board of Nursing,

  • UK HealthCare,

  • and others, such as attorneys!
